If you’ve noticed that your brow overshadows your eyes more and more as you’ve gotten older, or your face seems angry or tired even when you’re in a good mood, you could benefit from brow lift surgery. A brow lift, also known as a forehead lift, helps brighten your face by lifting your eyebrows for a more open, refreshed look.

What Is a Brow Lift?

With age, the wide forehead muscles and supportive tissues gradually weaken, which, helped along by gravity, causes the skin and soft tissue of the brow area to gradually lower. This combination of factors can lead to a heavy, overhanging brow or the appearance of being constantly tired or stressed, even when your face is at rest. In some cases, a sagging forehead can even interfere with normal facial expressions and can contribute to the deepening of existing forehead wrinkles and excessive hooding over the upper eyelids.

A brow lift repairs and tightens lax muscles for a smooth, unlined appearance across the forehead, minimizing pronounced horizontal wrinkles and restoring a natural arch to the eyebrows. This helps to create a feeling of openness through the upper face so you look younger, less worried and more rested.

Are You a Candidate?

The best candidates for a forehead lift are those men and women with lowered eyebrows, deep forehead wrinkles and other signs of advanced aging through the brow and forehead area. It’s also important to be in good overall physical and mental health, and to have realistic expectations about your results.

A brow lift can help you look younger, but the results are localized to the forehead. If you have concerns such as sagging eyelids or crow’s feet in addition to a fallen brow, we may recommend including additional procedures such as eyelid surgery or BOTOX® to further enhance the results of your brow lift. Brow Lift Techniques

There are a few different forehead lift techniques that may be used during the surgery:

  • Open (Coronal) Technique: In this traditional approach to lifting the forehead, your surgeon will place the incision across the top of the scalp, beginning just above each ear and hidden well within the hairline.
  • Endoscopic Technique: An endoscopic brow lift allows for smaller incisions and a less invasive surgery, which means faster healing and recovery. In an endoscopic brow lift, small incisions are required. Three incisions are typically placed along the top of the scalp, along with one at each temple, behind the hairline.
  • Trichophyic Brow Lift: This technique involves placement of the incision just inside the hairline. This technique can actually be used to lower a high hairline. Initially the incision may be visible but when healed it is usually imperceptible. This technique is generally less uncomfortable than the endoscopic or coronal brow lifts.
  • Limited Incision Technique: In a limited incision brow lift, your surgeon hides the incisions within the hairline at the temples. Alternatively, he can perform a brow lift using the same incisions as required during upper eyelid surgery if the procedures are performed at the same time.

Brow Lift vs. Eyelid Surgery

While brow lift surgery and eyelid surgery both rejuvenate the upper face, they address different concerns. A brow lift focuses on raising drooping eyebrows and smoothing forehead lines, which can open up the eye area and correct a heavy or tired appearance caused by low brows.

Eyelid surgery, also called blepharoplasty, targets excess skin, puffiness or bags on the upper or lower eyelids. It improves the appearance of the eyelids themselves but does not reposition the brows. In some cases, patients can see the best results when combining both procedures to achieve a more refreshed, balanced result across the entire upper face.

Your Brow Lift Consultation

The first step in your brow lift journey is a consultation with Dr. Koehler or Dr. Gardner. During this visit, your surgeon will evaluate the natural movement of your forehead and brows and talk with you about the concerns you would like to address through facial rejuvenation surgery.

Together, you’ll review whether a brow lift is the right solution or if another approach would better meet your goals. Your surgeon will walk you through the recommended technique, what recovery looks like and the kind of changes you can realistically expect, so you can make confident, well-informed decisions about your treatment.

The Procedure

A forehead lift is typically performed as an outpatient procedure under general anesthesia, allowing you to return home the same day. The surgery usually takes 1 to 2 hours, depending on the technique used and whether it’s combined with other facial procedures. You’ll need a responsible adult to drive you home and stay with you for the first 24 hours.

During the procedure, your surgeon carefully repositions the brow to a more youthful, natural position while minimizing visible scarring. Once surgery is complete, you’ll be monitored briefly before being discharged with detailed aftercare instructions to support a smooth recovery.

Brow Lift Recovery

Most brow lift patients are able to return to work within 10 to 14 days after their procedure. You may apply makeup around the two-week mark to help conceal any lingering bruising or swelling, though it’s important to check with your surgeon before using products near incision sites. Sleeping with your head elevated for the first few nights can improve comfort and help reduce swelling.

Strenuous exercise and physical activity should be avoided for several weeks. Your surgeon will guide you on when and how to gradually resume workouts based on your healing progress. Antibiotics may be prescribed to help prevent infection, and sutures are typically removed about 1 week after surgery.

Some swelling and bruising are expected early on, but these usually resolve within 1 to 2 weeks, around the same time most patients return to their normal routines. As healing continues, results look increasingly natural, with many patients noting they appear more refreshed, approachable and well-rested.

Your New Look

You should see an improvement in your facial appearance immediately after your brow lift, although initial swelling and bruising can disguise your results at first.

As swelling continues to subside, your results will continue to become more defined and noticeable. Most patients see their final brow lift results within 3 to 6 months, once healing is complete and the brows have fully settled into their new position.

Deep horizontal forehead furrows should be diminished or resolved, and your upper face should look more open and alert. The arches of your eyebrows will be restored to their natural curve instead of seeming flat and horizontal, and your facial expressions should look normal and untroubled, along with a more youthful appearance overall.

However, a brow lift may be combined with other procedures, such as an eyelid lift or facelift, if other cosmetic issues, such as drooping eyelids or lower-face wrinkles and creases, are also a concern.

What’s the best age for a brow lift?

There is no single “best” age for a brow lift. Candidacy depends more on facial anatomy, skin changes and cosmetic goals rather than age alone. Our patients commonly seek brow lift surgery from their 40s through 60s, though it may be appropriate earlier or later depending on individual needs.

Does a brow lift hurt?

You shouldn’t feel any pain or discomfort during your brow lift, as it is performed under either general or local anesthesia. Most people find that this outpatient procedure is fairly straightforward to recover from with minimal pain. However, some discomfort, tenderness, swelling and bruising are to be expected during the initial stage of brow lift recovery.

Can a brow lift fix hooded eyes?

Yes, a brow lift can improve hooded eyes when the heaviness is caused by low or sagging brows. If excess skin on the eyelids is the primary issue, eyelid surgery may be recommended instead, or in combination, for the best result.

How much time will I need to take off work to recover from a brow lift?

Most people are able to return to work within 10 to 14 days after getting a brow lift. You should be able to apply makeup around two weeks after your brow lift to help conceal any residual swelling or bruising. However, check with your cosmetic surgeon before applying any topical products to areas with incisions.

You will need to avoid strenuous exercise or physical activity for several weeks after your brow lift. Work with your cosmetic surgeon to gradually reintroduce exercise to your daily routine.

How soon will I see results from a brow lift?

You may notice an immediate difference in the appearance of your forehead and brow directly after getting a brow lift. However, swelling and bruising can hide your initial results, so you may not see a significant improvement until around 1 to 3 months after your procedure. It can take six months or longer to see your final brow lift results, so remain patient as your body heals and continues to change.

Will I have visible scars?

Incisions from a brow lift are placed discreetly within the hairline or natural creases to minimize visibility. Any scars typically fade over time and are well concealed once healing is complete.

How long does a brow lift last?

Generally speaking, a brow lift can last 10 years or more. However, the longevity of your brow lift will depend on a number of factors, including your age, genetics, the elasticity of your skin and lifestyle habits. You can help to prolong your brow lift results by avoiding sun exposure, wearing daily sunscreen, eating a healthy diet, drinking enough water and managing stress.

Is a brow lift covered by insurance?

A brow lift is usually considered a cosmetic procedure and is therefore not typically covered by insurance. However, in cases where drooping brows significantly impair vision, full or partial coverage may be possible, but this requires medical documentation and approval from your insurance provider.
